Intensive Case Management and Blended Case Management services are available for individuals with mental illness residing in Queens and Brooklyn. Adult Home Case Management is offered to residents of the Anna Erika Adult Home in Staten Island.

Case management teams provide assistance to individuals in identifying and accessing a range of health and mental health resources. The primary goal is to help individuals achieve the maximum level of independence in the most appropriate and least restrictive environment.
