PSCH offers three Assertive Community Treatment (ACT) teams: in the Bronx, Brooklyn and in Queens. These multidisciplinary teams provide social work, rehabilitation, counseling, nursing and psychiatric services, employment and housing assistance, family support, substance abuse services, and other assistance to help individuals with a diagnosis of mental illness or co-occurring substance abuse issues to live successfully in the community.
